The Home care nurses visit discharged patients within 24 hours of their discharge to follow up on their wellbeing. Additional to this, Post-natal patients have a revisit after a fortnight to ensure proper wound healing and lactation being established. The neonates are assessed for neonatal jaundice. Patient seeking emergency care are visited by the cardiac team (ACLS trained doctors and nurses) and care is provided as per the treatment protocol. As part of the post-natal visit, the Home care Nurse provides information on the importance of breast feeding. She informs the mother on the advantages of breast feeding to herself and her baby. The nurse encourage and train the mother how to manage Breast engorgement .She plays a significant role in helping the mother to begin breast feeding and to enjoy it, at the same time providing her infant with optimum nutrition for his/ her early growth and development. During her visit she assesses both the mother and baby, if any abnormal findings she advice and carry out the treatment as per the standing orders. In case the identified problem is not manageable she will inform the respective consultant and advise them to do a follow-up.
