Overview

Dr L H Hiranandani Hospital has a "round-the-clock" laboratory testing facility catering to both outpatient as well as patients admitted in the hospital with an extremely comprehensive menu of investigations to choose from which includes routine and specialised investigations. The Department of Laboratory Medicine is equipped with the latest state-of-the-art analyzers, which are fully integrated with the Laboratory Informatic System providing for online patient results. The department boasts of consistent and quality results through stringent Quality Control measures and participation in recognised Proficiency Testing Programmes. The department has been accreditated (MC-2145) in the field of medical testing by the National Accreditation Board for Testing and Calibration Laboratories (NABL) for international standard ISO 15189: 2022. Clinical Biochemistry

The Vitros XT7600 biochemistry analyzers are equipped with primary tube sampling with sample identification via bar code scanner system. The analyzers are capable of handling maximum theoretical throughput upto 1320 test results per hour respectively with the capacity for processing urgent (STAT) samples along with the batch analysis, thus making critical information available quickly for emergency samples. The patented spreading layer of the microslide technology minimises the interferences from hemolysis, lipemia and bilirubin thus improving the integrity of the result. The Vitros XT7600 is equipped with following technology:

  • Microsensor Scan for sample interferences (hemolysis, icterus, turbidity)
  • MicroTip
  • Intellicheck™ Technology includes clot and bubble detection, Save-the-Sample Clot Management, liquid level sensing and reagent delivery detection and management

 

The Cobas b221 automated blood gas analyzer from Roche has following salient features

  • Measures Total Bilirubin (useful in the neonatal cases), Ionized Calcium and Lactate levels
  • Haemoglobin derivatives such as oxy-Hb, reduced Hb, methaemoglobin and carboxy-Hb are measured
  • Patient Trending – provides for historical trending of patient parameters simultaneously – PO2, PCO2, pH, glucose, lactate, co-oximetry, sodium, potassium, chloride, and ionized calcium – this will help to follow the progress of patient's condition during the course of treatment.
  •  Acid-Base Map – help in precise diagnosis and therapy mentoring; identify metabolic and respiratory acid-base disturbances.

The Biorad D-10 Haemoglobin testing system provides comprehensive haemoglobin testing in a compact footprint. This fully automated system measures HbA1c levels for the diabetes monitoring. The features of the system include primary tube sampling with cap piercing, barcode handling for positive sample identification and printed chromatograph. The cation-exchange high performance liquid chromatography (HPLC) technology used for measurement is the reference method for haemoglobinopathy thus ensuring maximum accuracy.

 

Immunoserology

The section of immunoserology offers an exhaustive menu of investigations covering hormones, infectious disease, oncology, cardiology, and anemia.

The immunodiagnostic equipments, XT7600 system and Architect i1000SR, provide marked improvement in quality of results, optimizing assay performance and productivity. The continuous, random access to the sample loading alongwith STAT operation helps in providing results any time. The systems provide for the positive sample identification and results integrity verification as well as extensive immunological menu. Various patented technologies in the systems perform, monitor and verify diagnostic checks throughout sample and assay processing resulting in increased confidence in result reporting. The methodology is chemiluminescence technology and this provides for accurate and highly specific results.

The automated immunoanalyzer Vidas Blue is a compact system for immunoassay using enzyme-linked fluorescent assay (ELFA).

In addition to routine tests like Widal, VDRL etc, serological investigations for "difficult to grow" pathogens as Leptospira, Dengue, Rotavirus etc., are performed by enzyme immunoassay and latex agglutination techniques.

 

Haematology

The fully automated haematology analyzers, Sysmex XN1000 perform comprehensive complete blood count using innovative fluorescent flow cytometry technology. The haematology analyzers are a 6-part differential standard and provides –  

  • NRBC testing on every CBC; thus reduces the need for reflex testing or performing automated/manual differentials
  • Corrected WBC count in presence of high NRBC results
  • Automated reportable immature granulocyte count (IG#, IG%) with every automated differential reduces manual review rates
  • Fluorescent Platelet Technology gives Platelet-F along with IPF which greatly reduces interferences from RBC or WBC fragments
  • Fluorescent Reticulocyte Technology using flow cytometry generates reportable Reticulocyte count, Reticulocyte Haemoglobin equivalent (RET-He) and Immature Retic Fraction (IRF)

The equipments have integrated with comprehensive delta-check flagging helping generate interpretative data, thereby warning about abnormalities of blood cells. All such 'flagged' samples are then visualised by the Pathologist to confirm diagnosis.

The CA-600 system is a compact, fully automated coagulation analyser which utilizes the proven optical detection technology and is a complete workstation for haemostasis & coagulation studies.

 

Clinical Pathology

The fully automated urine analyzers, UN2000, combines the UC3500 chemistry module with the UF5000 urine particle module.

UC3500 chemistry module: The entire sequence, starting from sample aspiration to colour comparison and final output of the results is fully automated. The new CMOS sensor enables the device to effortlessly distinguish abnormal colors between RBCs and hemoglobin. The refractometry measurement method enables accurate results for specific gravity and turbidity. The fast analyser is capable of processing 276 samples per hour.

UF5000 urine particle module: Instrument uses 2 different channels like Core & Surface and cells are differentiated as nucleated & non-nucleated cells.

 

Microbiology & Molecular studies

The microbiology lab is well equipped with sophisticated systems such as BacT Alert and VITEK 2 automated systems.

The BacT Alert 3D microbial detection system is an automated test system capable of incubating, agitating and continuously monitoring aerobic and anaerobic media inoculated with patient specimens suspected of having bacteremia, fungemia and / or mycobacteremia. The system utilizes barcode technology to assist in specimen and data tracking. The BacT Alert MB configuration is designed to rapidly grow and detect mycobacteria from patient samples.

With an extensive database capable of identifying over 330 species of microorganisms, the VITEK 2 Compact Microbiology System provides same-day identification for 95% of routine organisms. Integrated in the VITEK 2 Compact system is the Advanced Expert System (AES™), software that validates and interprets susceptibility test results, and detects antibiotic resistance mechanisms. The AES Expert System is the most developed software system in this field, and is capable of identifying even emerging and low-level resistance.

MGIT :The BD BACTEC™ MGIT™ automated mycobacterial detection system is a fully automated solution for mycobacterial liquid culture and susceptibility testing.

  • Continuous monitoring: The system identifies positives as they occur, speeding results to improve patient care, reduce hospital stays, optimize equipment, and staff utilization.
  • Onboard incubation: Safe and efficient onboard incubation eliminates sharps, and the need to handle or transfer tubes once the system is loaded, reducing labor requirements while adding a layer of operator safety.
  • Nonradiometric technology: The system uses BD BBL™ MGIT™ media and patented sensors, which efficiently leverage advanced fluorometric technology to facilitate highly accurate detection of O2 consumption without sharps.

 

GenXpert (PCR-CBNAAT)

The GeneXpert System runs PCR (CBNAAT) testing on 4-module configurations which use the same patented cartridge technology for every Xpert®test.

  • Highly accurate PCR results for consistent patient management across the continuum of care
  • Fully automated sample extraction, PCR amplification, and detection
  • Fast, actionable data with most test results in less than one hour and respiratory test results in as little as 30 minutes

 

QIAstat-Dx assay (Multiplex PCR technology)

The QIAstat-Dx assay cartridges use multiplex PCR technology – also known as syndromic testing – to quickly survey many different pathogens in a single patient sampleIntuitive workflow with minimal hands-on real-time PCR to detect pathogen nucleic acids in human biological samples. It uses real-time PCR to deliver comprehensive results in about an hour. Easily viewed Ct values and amplification curves for all detected pathogens are available for better interpretation.

 

Apart from routine isolation & identification for aerobic organisms of all clinical samples, anaerobic, fungal & mycobacterial cultures are also done from the clinical samples.

The active microbiology surveillance of operation theatres & high-risk areas in the hospital forms part of the protocol of the hospital infection control. The hospital infection control committee lays down the policy for use of antibiotics, disinfectants & waste disposal.

 

Histopathology and Cytopathology

The section offers services for processing of clinical specimens for histopathology as well as reporting of fluids, smears and fine needle aspirations. In addition, 'frozen section' service is provided for urgent diagnosis of surgical cases. All surgical specimens are processed by standard histological technique and reported with diagnosis, grading and staging of tumours according to international guidelines. The section also handles both gynaecological and non-gynaecological specimens sent for cytology. The gynaecological 'PAP smears' is examined for early detection of cervix cancer. Fine needle aspiration cytology (FNAC) is also carried out for all palpable swellings in the body as the first line of investigative workup before a surgical biopsy.
